To me, only intensive studying with a native can help you. I mean not "academic style" learning, but kind of speaking club format (when you discuss anything you like and are corrected a little in the process)! It's the key! Cause neither drilling grammar nor cramming vocabulary helps! Overcoming speaking barrier is the essential aspect!
P. S. When my students ask me the same question, I always give this example... Think of millions people in Ukraine, Belarus, etc. (where communicating in Russian is so common). Most of them understand Ukrainian/Belarussian, but very little speak (I mean quickly, automatically, confidently!). Furthermore, lots of them graduated from linguastic universities, have deep understanding of all the difficult grammar stuff, know all the advance language nuances! But they still CAN'T SPEAK (quickly, automatically, confidently) on every day topics!
So, if you wanna learn how to speak - just SPEAK! :)
