Menstruation, as far as I recall, is the shedding of the uterine lining after ovulation, so clots are actually the very essence of "those" days, while blood is merely an "addition".

It's simply that your endometrium has built up quite significantly, which is shed each month.
Due to certain hormonal shifts, the endometrium can grow up to 2.5cm in thickness. Imagine, the uterus in its normal state is about the size of a fist, and inside there's a cavity (like in your fist, but in cross-section, the uterus somewhat resembles an upside-down pear – with the base at the top and the tip pointing down). Now picture that inside this cavity, a lining has built up to 2.5cm (for some it's less, for others more). And this "delight" has to be shed. It can come away in pieces, or it might all come out at once, however it happens. There's no need to be afraid, but do keep up with your gynaecology check-ups.
As for fibroids, as mentioned here, they have absolutely no relation to the endometrium in this case.
