Severely elevated ESR

Hello everyone! I'm writing on the forum with this question. I had a medical check-up at work, and according to the blood tests, the ESR indicator is severely elevated, 40. The normal is up to 20. Everything else is normal. The doctor asked me what I'm sick with, why it's so high. She told me to retest in about 2 months. Currently, I'm not sick with anything. I've read online that high ESR can occur with cancer, autoimmune diseases, even with HIV. I've already worked myself up to the point of panic. Please write if anyone has had such a high value, what the outcome was. Thank you.

Author, don't jump to conclusions just yet. ESR is an indicator of inflammation in the body—it can even rise with appendicitis or pneumonia. Ask your primary care physician for a referral to an immunologist or retake the test at a private clinic. At my clinic, my CA 15-3 tumor marker was elevated, and I thought it was all over—time to write a will—but a retest elsewhere came back fine. It really makes you wonder how they handle these tests in some labs. Even doctors themselves admit that labs often perform poorly (for instance, my endocrinologist told me they sometimes assign TSH values arbitrarily). Don't stress out for now; calm down. If you're constantly feeling unwell, experiencing fatigue, and losing weight, that's one thing. But if you're a perfectly healthy, vibrant young woman, that's another, and there's no reason to worry yet. I would retest at a private facility (but only one you trust—ask around for recommendations) and, most importantly, push for a referral to an immunologist. Many people have weakened immune systems these days, which can lead to all sorts of irregularities in test results.

Well, if she went through the medical check-up, then her biochemistry and HIV, hepatitis, syphilis tests were definitely done. If only the ESR is off the charts, then for doctors that's usually not a signal to take action.

Something serious. You should also get checked for tuberculosis. Even with my chronic cystitis, adnexitis, and autoimmune bowel disease, my ESR is normal.

An elevated ESR is a likely sign of inflammation. 40 is not that alarming. It could be due to a recent respiratory infection.

Don't worry and just retake the test. With serious diagnoses, the ESR is much higher.

Go to your general practitioner and get a comprehensive blood test (it's better to do it on your own before seeing the doctor so you can come with the results ready), as it will already show the approximate cause.

I have had a permanently elevated ESR all my life; at most, doctors would just mention it's above normal, and often they completely overlooked it. It wasn't until about 15 years later that it became clear I have chronic tonsillitis.

From my bitter experience, I can say that if hemoglobin is low and ESR is high, it could very well be cancer. That was the case with my sister, my husband, and me. But if hemoglobin is normal, while ESR is high and white blood cells are above normal, then it's likely an inflammatory process somewhere.

Not necessarily. It could be gastrointestinal issues, elevated bilirubin - low hemoglobin.

Same here! I take hormones too! ESR 37, 40! Therapists stay silent because all other blood indicators are normal! Well, my knees hurt! I keep wondering too! Why is the reading so high?

But back in the USSR days, an ESR of 70 was a huge red flag! They admitted me to the hospital within 2 hours—turned out I had thyroid inflammation. I was hospitalized for a month.
